• Formal application letter to be submitted stating your request. Include your current address and telephone number, please.
• Submit any pension form to be filled out by the Embassy (if applicable).
• Copy of your Nigerian passport.
• Copy of State-issued U.S. valid Identification (i.e. Driver’s License or I.D card).
• Copy of retirement letter or offer of appointment letter.
• A certified return Prepaid Mailing Envelope from USPS, UPS, or FedEx (if applicable).

NOTE: Allow 7 working days for processing time. Also, drop off and collection of documents from Monday through Thursday between 11:am to 12:30 pm.
